I．はじめに

　急性硬膜外血腫は，一般に骨折を伴ったcoup injuryとして見られることが多い^14）．一方で，骨折を伴わない症例が，急性硬膜外血腫全体の11-15％^4,9）にも及び，若年層を中心に見られる^4,9）ことも事実である．また，両側性急性硬膜外血腫も剖検例の報告で2.4-5.4％^5,6,21）に見られたとされ，手術による救命例の報告^7,8,20）も散見される．

　今回われわれは，左後頭部を打撲，同部位に骨折を認め，右前頭部に急性硬膜外血腫を形成した，稀な1例を経験したので，発生機序を中心に文献的考察を加え報告する．

Acute extradural hematoma, a collection of blood be-tween the inner table of the skull and the dura, general-ly occurs in coup craniocerebral trauma with skull frac-ture.

We report here a case of a 52-year-old female who had had a traffic accident. She suffered a serious wound consisting of a subcutaneous hematoma with skull fracture and so called Battle's sign on the left occipital region but not wounded in any other region.
