I．緒言

　脳腫瘍患者に対して脳室腹腔短絡術およびその他の髄液内誘導術によるshunt systemの設置は，腫瘍による髄液循環障害により水頭症を伴う場合にしばしば施行されている．頭蓋内原発腫瘍の神経管外転移は稀であるが，この人工髄液路を介して脳腫瘍が神経管外へ転移した症例は文献上30例程報告されている^{5,10,15,21,24,27,30）}．このshunt systemを介して神経管外へ転移するものは髄液播種をきたしやすい腫瘍が比較的多く，medulloblastomaやgerminal tumorなどが挙げられる．一方gliomaは，特に手術など人工的な播種経路の作成が加えられない限り，神経管外に転移することは極めて稀である．また人工髄液路装着後でも，その誘導部位に転移巣を認めた報告は現在まで文献上8例の報告を認めるに過ぎず，しかもその多くは死後剖検により確認されている．今回われわれはmatignant gliomaの症例に脳室腹腔短絡術施行後わずか1ヵ月で腹水貯溜を認め，腹水型gliomaの病像を示した極めて稀な症例を経験したので報告する．

This is the first report of extraneural metastasis of malignant glioma through V-P shunt tube and growth in peritoneal cavity as ascitic form. The patient was a 43-year-old man who was admitted to our hospital with occipital headache. CT scan showed enhanced cystic tumor mass at left temporal lobe. Craniotomy and partial excision of the tumor was done and the histology of tumor tissue showed a malignant astrocytoma. Following this treatment, the patient received the adjuvant therapies of radiation, chemotherapy and immunotherapy with interferon, and also recraniotomy three times.
