神経管外転移により気管支閉塞症および上大静脈症候群を合併したMalignant ependymomaの1例—症例報告および文献的検討

Extraneural Metastases of Malignant Ependymoma which induced Atelectasis and Superior Vena Cava Syndrome: A case report and review of the literature

I．緒言

　従来，神経系腫瘍の神経管外転移は少ないとされてきたが，近年の診断技術の向上および積極的な手術療法による人工的侵襲の増大，さらには免疫化学療法の改善による脳腫瘍の長期生存例の出現とともに，転移症例の報告が相次いで認められてきている．脳腫瘍の好発転移部位は，腫瘍種別により異なるとされているが，一般には頭頸部リンパ節から骨，肺などへの転移が多いとされる．しかし，現在までの報告では死後剖検にて転移巣が発見されることが多く，生前に転移巣を確認できた症例は比較的稀である．さらに転移巣が直接の死因となった症例は極めて稀と考えられる．当教室では，最近，悪性神経膠腫の症例に，両側肺門リンパ節転移を併発し，上大静脈症候群および気管支閉塞症をきたして，呼吸不全にて死亡するという，転移巣が直接の死因となった極めて稀な症例を経験したので報告する．

Extraneural metastasis of intracranial ependymoma is a rare pathological entity. Thirty one case reports were traced in the review of the literature and we record one of them.

The patient was a 19-year-old male in good health until January 1981 when he was admitted to our hospital with deteriorating mental status. Admission work-up revealed bilateral papilledema, 1-hemiparesisand increased intracranial pressure signs including vomiting. CT scan demonstrated significant abnor-mality of enhanced mass lesion in the r-temporo-parietal area in which a displacement of the midline structure to the left occured.
