I．はじめに

　三叉神経は橋腹外側より出て，テント下面よりMeckel腔に入るが，この都位に腫瘍が生じた場合，小脳橋角部はいろいろな腫瘍の好発部位でもあるので，他の腫瘍との鑑別が必要となる．三叉神経腫瘍は，腫瘍が三叉神経より出るにもかかわらず，大きくなるまで症状が出現せず，かなり大きくなりてから発見される場合が多い．CT scan出現以前でも中頭蓋窩にみられるGasserian ganglion neurinoma, dumbbell型neurinomaは容易に発見されたが，trigeminal rootneurinomaは診断が困難であった．後頭蓋窩neurinomaはCT scan上よく造影されるので発見は容易になったが，頻度の高い聴神経腫瘍他のcp angle tumorとの鑑別が必要である．われわれは従来の神経放射線検査のほかに神経耳科検査を補助手段として総合的に鑑別診断，障害の部位診断も行っているので報告する．

The authors report a case of neurinoma arising from the trigeminal root and describe the differential diagnosis and the operation of cp angle tumor in the era of CT scan. A 64-years-old male physician was admitted to our service, Nagoya University Hospital with 10 months duration of gait disturbance, 9 months history of hvpoesthesia in the right side of his face and tongue, four months history 01 hard of hearing in the right side and of speech disturbance. He had noticed increased staggering gait associated with gait deviation to right and dysarthria a few months prior to CT scan.
