I．はじめに

　脳膿瘍は，抗生物質の発達による肺炎・腹膜炎・髄膜炎など各種感染症の死亡率の著しい改善にもかかわらず，その治療成績は必ずしも向上せず予後不良な疾患とみなされてきた^{1,2,4,12,22,33,38）}．しかし近年CTの導入により本疾患の治療成績は著明に上昇してきている^{5,7,11,14,18,21,24,26,27,33）}．

　最近われわれは過去1年間で種々の原疾患に続発する脳膿瘍の4例を経験した．その治療内容は，保存的治療2例・被膜外全摘術2例で全例軽快退院しており，良好な成績を収めている．そのうち1例は極めて予後不良といわれる脳室穿破例であった．これらのCT所見の特徴と治療に関して検討し若干の所見を得たので報告する．

The authors experienced with four cases of brainabscess, one of which ruptured into the lateralventricle.Two cases were treated conservatively and theremain-ing two were treated surgically. All cases recoveredsatisfactorily.

Case 1: A 30-year-old man with congenital cyanoticheart disease was admitted on Oct. 4, 1979, becauseofconvulsion. Physical examination revealed motoraphasia, right hemihypesthesia, cyanosis, clubbedfinger and continuous heart murmur. CT scan showeda ring-enhanced mass lesion in the left temporallobe.
