Ⅰ．はじめに

　現在においても脳膿瘍の治療成績は必ずしも満足すべき状態ではなく，治療面にもいまだ種々の問題を残している．なかでも膿瘍の脳室穿破は最も忌むべき合併症であるとされている．従来，脳膿瘍の脳室内穿破は脳室上衣炎，pan-meningoencephalitisなどをひき起こし，致命的な経過をとることが多かった^{2，6，8，13，18）}．しかしながら最近，治療の進歩によりさらに脳膿瘍の膿にはsterile pusが多くなり，穿破した脳膿瘍のとる病態にも変化がみられている．

　脳膿瘍の脳室内穿破に関する報告は比較的少なく，特に治癒例の報告は著者らの知る限り，本例を含めて8例^{5，14，15，20）}ときわめて少ない．著者らは最近，脳室内に穿破した脳膿瘍を全治せしめた1症例を経験した．この自験例の脳血管撮影所見，conray cistogram,EMIスキャン所見を中心にのべてみた．

The clinical features and course of patient who survived from rupture of brain abscess into the lateral ventricle are described. The carotid angiogram, conray cystogram and CT scan are shown.

A 50-year-old female admitted to our hospital, with few days history of headache, confusion and left hemiparesis. Right carotid angiogram demonstrated a large frontal mass. Comity cystogram showed leakage of contrast medium from frontal lobe abscess cavity into the lateral ventricle.
