Ⅰ．はじめに

　1885年，Oslar^18）が動脈壁の細菌感染に由来する動脈瘤をmycotic aneurysmと命名して以来この名称が一般に用いられるようになったがこれは比較的まれな疾患であり，そのほとんどがendocarditisに合併する．最近われわれは先天性疾患であるsupravalvular aortic stenosisを有する患者の右中大脳動脈に発生したmycotic aneurysmと同時にbrain abscessを合併した症例を経験した．これら両者はendocarditisの合併症として知られているが同時発生例の詳細な報告はいまだみあたらない．mycotic aneurysmの手術適応については見解のわかれるところであるが，われわれはこの症例を手術的に治癒せしめえたので若干の文献的考察を加えて報告する．

A curative case of intracranial mycotic aneurysm with brain abscess, which occured as a complication of bacterial endocarditis, was reported.

A 25-year-old female with a congenital supravalvular aortic stenosis was admitted to another hospital because of severe headache, vomiting and convulsion on Nov. 5, 1976. Right carotid angiography revealed a dumbbell shaped aneurysmal shadow at the distal portion of middle cerebral artery. Occulsion of major branches were also seen in the middle cerebral artery. She was referred to Tohoku University Hospital on Nov. 19, 1976.
