I．はじめに

　CTを用いた定位的脳内血腫除去術は，Backlundら^1）により始められ，その後，林ら^4），駒井ら^2），松本ら_7）の努力により，特に本邦にて発展・普及し，すでにその有用性は確立したといえる．このなかで，後頭蓋窩，特に小脳出血に対する定位手術については，アプローチの方法などでいくつかの問題点が残されている．

　著者らは，CT用定位脳手術装置を用い，CT室にて局麻下に定位脳手術を行っているが^12），今回，3例の小脳出血に対して，仰臥位にて頭部を健側に30-40°傾け，側方からアプローチすることにより，比較的容易に血腫除去を行いえたので報告する．

Stereotaxic lateral approach for cerebellar hematoma is presented using Leksell's CT-stereotaxic system.

All of the procedures are performed in the CT room. Patient's head is turned to contralateral side of the hematoma 30 to 40°with slight flexion of the neck. Stereotaxic apparatus is secured to the head under local anesthesia. Hematoma is confirmed by com-puterized tomograms. Three dimensional coordinates of the target point (center of the hematoma) are mea-sured from the vertical and diagonal rods of Leksell's system. Linear skin incision 4 cm in length is made on retromastoid area.
