I．はじめに

　1973年にcomputerized tomography　（以下CTと略）が世に出て以来，多くの研究がなされ，その診断学における重要性は計り知れない．しかし一方ではCTに頼りすぎて，神経学的所見，臨床所見等を軽視しがちになることもいなめない．CTによる診断と，臨床所見あるいはその他の神経学的な補助診断法による診断との不一致は日常多々経験するところである．われわれは臨床所見およびCTを含む神経放射線学的検査において脳腫瘍と診断し，開頭術を施行したが，組織学的に腫瘍は証明されず，1カ月後と3カ月後のCTでは正常像を呈し，抗痙攣剤投与により無症状となった症例を経験したので，今後のCTによる診断学の発展に何らかの役に立つことを願い報告する．

A 24-year-old male was admitted with a 4-month history of focal seizures of the right arm with postictaparesis. On examination, he was alert and cooperative, and had noproblem in speech. There was no sign of raisedintracranial pressure. He exhibited frequent focal seizures of the right arm followed by postictal pareses, affecting more the proximal part than the distal part of the arm. He also noticed dysaesthesia of the right arm. The neurological examination, however, revealed no sensory deficit. Plain skull films were negative. EEG showed some slow wave components over the left side of the head.
