椎骨脳底動脈の解離性動脈瘤

Dissecting Aneurysms of the Vertebro-vasilar System:Cliniacl and Angiographical Observations

I．はじめに

　これまで脳血管の解離性動脈瘤は，頭蓋内外を問わず比較的稀れなものであるとされてきた．特に椎骨脳底動脈領域の解離性動脈瘤の報告は少なく，調査いえた範囲では28例（非外傷性のみ）にすぎない^{1-7,9,10,13,14,16,17,20-33）}．最初の報告例はScholenfield（1924）^25）による47歳男性の剖検例であった．更にこれら28例のうち25例は剖検により診断されたものであり，臨床診断がなされたものはわずか3例（われわれの2例を含めて5例）にすぎない．このうち，Ouchiら（1965）^20）の例は椎骨動脈起始部の動脈瘤と診断され，手術による切除標本から確定され，Wagaら（1978）^29）の例はsaccular aneurysmとされ手術所見から診断された．血管撮影により診断された例はFischerら（1978）^10）の1例のみである．われわれの2例は血管撮影により診断された第2，3例目にあたる．

　文献例を渉猟すると内頸動脈領域を含めても脳血管の解離性動脈瘤の報告はたしかに少ないのであるが，従来考えられているほど少ないものではないという報告もある．Ehrenfeld ＆ Wylie（1976）^8）は10年間に19例（いずれも頸動脈領域）を，Fischerら（1978）^10）は，22例（うち1例は椎骨動脈）をいずれも脳血管撮影上発見したと報告い血管撮影上の所見を詳細に述べている．

Spontaneous dissecting aneurysms of cerebral arteries, especially of the vertebro-basilar system, have been rarely reported in literature. Two cases which were angiographically diagnosed and successfully operated were added to 28 cases previously described. These two cases were 44 year-old male with numbness and motor weakness on the left upper and lower extremities, and 52 year-old male with recent memory disturbance after a episode of subarachnoid hemorrhage.

Dissecting aneurysms of the vertebral and basilar arteries were angiographically demonstrated as shown in the figures.
