I．はじめに

　耳下腺局所症状に乏しく，後頭蓋窩の1／3を占める耳下腺原発の悪性混合腫瘍のために一側脳神経が広汎に障害され，のちに頸椎に転移した症例を経験した．その症状，経過は非定型的であり，診断は困難で，最終的には組織診で確定した症例について報告し，頭蓋外原発腫瘍の頭蓋内浸潤発育の存在に脳神経外科医の注意を喚起する．

Invasive growth of malignant parotid in the posterior fossa was found in a 58 years old farmer. He had suffered from left facial palsy, left deafness and tinnitus for a year and recently dysphagia.

He admitted on Sept. 21, 1979. He had lost considerable weight and showed dehydration. Neurological examination revealed the following: The eyes at right and left gaze showed horizontal nystagmus. Examination of the eyegrounds disclosed 1 D. of bilateral choked discs. Perception of touch and pain was diminished on second and third branches of left trigeminal nerve.
