Ⅰ．序論

　頭蓋咽頭腫は小児脳腫瘍の代表的なものの1つであり，その臨床像は頭蓋内圧充進，眼症状，内分泌障害および頭蓋レ線上の石灰化像が特徴的である^1,16）．一方，成人例を含めても本腫瘍が運動麻痺を呈することは極めて少ない^{1,2,7,18,19）}．これは頭蓋咽頭腫の発育方向が主として上方進展を示し，第3脳室圧迫の傾向を取るためと理解される^19）．

　著者らは最近，片麻痺を主訴として来院し，諸検査および手術によって，側方進展を主とした巨大な頭蓋咽頭腫であることを確認した小児例を経験したので，確定診断までの経緯を中心に報告する．

A rare case of hemiparesis associated with a craniopharyngioma of 8-year-old child was reported. She was first seen at The Department of Neurological Surgery, Kitasato University Hospital, in May, 1979. with a chief complaint of hemiparesis on the left side for 2 months. She had not complained of visual disturbances.

Neurological examinations on admission disclosed hemiparesis with hyperrefiexia on the left side. The remainder of the neurological examination give negative results. Visual disturbances on admission could not be exactly obtained since she was not cooperative.
