頭蓋咽頭腫により内頸動脈狭窄を示した1症例

Stenosis of the Intracranial Internal Carotid Artery by at Craniopharyngioma:Report of a case

I．はじめに

　頭蓋内腫瘍，特に髄膜腫^{1,6,10,15,16,22,23）}，下垂体腫瘍^{2,4，19,21,23）}に起因する頭蓋内内頸動脈の狭窄および閉塞はしばしば報告されており，日常脳神経外科診療に際しよく遭遇するが，頭蓋咽頭腫^16）による内頸動脈の狭窄および閉塞の報告は少ない．

　われわれは巨大な頭蓋咽頭腫により内頸動脈の強度狭窄を来たした症例に浅側頭動脈中大脳動脈吻合術を行い，臨床症状の軽快をみたので，若干の文献的考察を加えて報告する．

Stenosis of the intracranial internal carotid artery byother tumors than meningiomas and pituitary adenomas isa rare occurrence. We present here a case with cranio-pharyngioma, which developed severe stenosis of the intra-cranial internal carotid artery 7 years following partialremoval and irradiation. This severe stenosis of the intra-cranial internal carotid artery would result from combina-tion of compression of the arterial walls against the sur-rounding bony structures and radiation effect on the arterialwalls.

A 29-year-old male was found to have a craniopharyngio-ma, which was treated by partial removal in 1971.
