白血病細胞浸潤による小児視神経腫瘍の1例

Well Marked Mass of Leukemic Cells around an Optic Nerve infiltrated by Leukemic Cells in a Child

I．はじめに

　眼窩腫瘍のなかでも眼球外腫瘍の場合には，頭蓋内との関係が深く，診断，治療の両面にわたり脳神経外科医の手に委ねられる機会が多い^17）．

　最近著者らは，急性リンパ性白血病（A．L．L．）完全寛解後の小児で，白血病細胞が頭蓋内くも膜，左視神経への浸潤に止まらず，左視神経周囲に頭蓋内から眼窩内へおよぶ腫瘤を形成し，手術に至った症例を経験した．

　本症例は非常に稀なものであるが，眼窩腫瘍の鑑別診断の際には常に念頭に置かねばならぬものであるため，若干の考察とともに報告する．

The authors had experience of a child with left orbital tumor. She had been treated as acute lymphatic leukemia in the past, but she was in complete remission at the time of this admission. At surgery a well circumscribed tumor was found around the enlarged left optic nerve both intracranially and intraorbitally. Histopathological study of the surgical specimen revealed tumor composed of leukemic cells and leukemic infiltration of the left optic nerve.

A 7-year-old girl was admitted to the Neurosurgical Division of Aomori Prefectural Hospital with exophthalmos and visual disturbance of the left eye.
