Ⅰ．はじめに

　脳内血腫の中で，外傷・高血圧・脳動脈瘤・脳動静脈奇形・血液凝固異常など明らかな原因を見出すことのできない，いわゆる特発性脳内血腫は，若年者に多く，出血部位が皮質下の白質内であり，予後が良いなどの特徴を有する1つのclinical entityとして扱われることが多い．近年こうした原因不明の特発性脳内血腫に脳血管写上もしくは病理組織上，small angiomatous malformation　（SAM）を証明する例が増加し，この疾患の呼称にも混乱がみられるが，今回は従来からいわれるように外傷・高血圧など明らかな原因を見出し得ない脳内血腫を特発性脳内血腫と解して，自験36例を集計分析し，その臨床像の多彩性を明らかにすると共に従来の補助検査による診断上の問題点，予後などにつき検討を加えた．

Thirty-six cases of so-called spontaneous intracerebral hematoma were analyzed with special reference to clinical signs and symptoms, computerized tomography (CT) scan findings and postoperative courses. Hematomas due to cerebral aneurysms, arterio-venous malformations, blood dyscrasias as well as traumatic and hypertensive intracerebral hematomas were excluded from this series.

The patients varied in age from 7 months to 74 years with the mean age of 40 years. There were 26 males and 10 females. Caution must be paid to the fact that the age group of more than 50 years occupied 37% of the whole cases.
