Ⅰ．緒言

　外傷性動静脈瘻のうち外頸動脈系より静脈洞への動静脈瘻は珍らしく，中でも中硬膜動静脈瘻は稀である．その報告も欧米，本邦併せても，1951年Fincher^2）が最初の報告をして以来，われわれの調べえた限りでは文献報告^{2，5，6，8，11，12，15-20，23，24）}20例，学会報告^{1，3，7，14，21）}7例，計27例であった．外傷性動静脈瘻は臨床的には自覚的な頭蓋内雑音，他覚的なbruit位で特有の臨床症状を欠く．しかし報告例の中には頑固な耳鳴りを訴えるものや硬膜下ないしくも膜下に穿破した症例もあり^17），外科的根治療法が望まれる，われわれはこれまでに2例の外傷性中硬膜動静脈瘻を経験したのでここに報告するとともに，若干の文献的考察を加えたい．

Case 1: A 38-year-old man who was involved in an automobile accident on February 7, 1971, was discovered unconscious on the road and was transferred to our department on following day. Left ear bleeding, semicoma, right hemiparesis and right extensor plantar reflex were noted in neurological examination.

Plain skull films revealed a left temporal linear fracture and separation of the left lambdoid suture. However, no hematoma was visualized in carotid angiography.
