Ⅰ．はじめに

　外傷性内頸動脈海綿静脈洞瘻（以下外傷性CCFと略す）は動脈血が直接海綿静脈洞内に流入する為に拍動性眼球突出，頭蓋内雑音及び時には大量の鼻出血を来たす疾患として古くから知られており^{35），その自然治癒は殆どないと言われている^26,30）．また本症の治療方法もTravers（1809年）^35）が患側総頸動脈結紮成功例を発表して以来，過去160年間，種々の外科的治療法が試みられて来たが，その多くは患側内頸動脈の血流を犠牲にするのみならず^{4,5,7,12,13）}，再発も多く治療効果が不確実なものであった．}

　今回我々はPolyurethane foam embolusを用いて所謂"タコ上げ法"^24,25,26）により脳血流の完全な温存のもとに，外傷性CCFを一次的に閉鎖し得たが，栓子作成法及び血管縫合法などの点で更に独自の改良を試みたので症例と共にその有用性について報告する．

A 29-year-old male was treated for a traumatic internal carotid-cavernous fistula by embolization with an improved type of captive polyurethane foam embolus which was inserted through the common carotid artery. Neither the intracranial nor cervical carotid artery was ligated.

The angiogram of right carotid taken one month after the surgery revealed satisfactory closure of the fistula and a perfect blood flow from the carotid artery involved.
