A 45-year-old female with traumatic carotid cavernous fistula on the right side was observed. The accompanying ophthalmoplegia was more severe on the contralateral, left side. A peculiar hemodynamic changes in the cavernous sinus and its surroundings seemed to be responsible for the apparent discrepancy.

The pupil reaction was intact throughout the course of the disease in spite of the total ophthal-moplegia on the left side. The sparing of the infe-rior oculomotor nerve branch appeared to be the cause of this phenomenon.
