Ⅰ．緒言

　下垂体腺腫の発育方向にはいろいろの方向のものが報告されているが実際には鞍横隔膜を挙上伸展し鞍上部に向けて発育するいわゆる鞍上部伸展がほとんどで，これ以外の方向，すなわちparasellar extension, frontal extension, temporal extension, posterior infratentorial extensionおよびPharyngeal extensionは比較的まれである．

　われわれは最近中頭蓋窩に巨大な腫瘤を形成し血腫を伴ったprolactin分泌性下錐体腺腫の1例を経験したので報告する．

The authors reported a case of prolactin secreting pituitary adenoma with unusual extension. A 21-year-old female was admitted to the Department of Neurosurgery, Hiroshima University Hospital on May 9, 1975 with the complaints of severe headache, nausea and visual disturbance. On admission she was slightly lethargic. The right eye was totally blind and there was a temporal hemianoptic visual field defect in the left eye. Fundoscopy revealed a right optic atrophy. The right pupil was mydriatic and did not respond to the light, but the extraocular movements were almost normal.
