Ⅰ．緒言

　脳下垂体腺腫内にはしばしば腺腫内出血が起こり，比較的小さい出血が腺腫内壊死およびそれにひき続く嚢腫形成を惹起すると考えられている．もし，きわめて大きな出血が起こって腺腫外にあふれ出した場合は，くも膜下出血の病像を呈し，激烈な症状を伴いこれが典型的な脳下垂体卒中（pituitary apoplexy）と称されている．もちろんこの両者の間に位する腺腫内出血も存在するわけで，このような場合には髄液内に血液は証明されないけれども急激な下垂体腺腫の体積増大によって頭痛，嘔気，嘔吐，視力障害の急速な進行，意識障害，ショックなど病態の急変，増悪を示す症状が突発する．

　このような場合に，臨床的にこれを典型的なくも膜下出血を伴うpituitary apoplexyと厳密に区別している人^1）もあり，pituitary apoplexyという言葉を比較的広義に解する人^2）もある．

1) It would be suggested that ACTH and TSH secretion of these cases was strongly impaired but LH, FSH and PRL secretion was fairly preserved immediately after hemorrhagic episode.

2) Two of these four cases revealed normal TSH secretion and normal thyroid function following the period of excessive TSH secretion and impaired thyroid function.
