Ⅰ．はじめに

　小脳出血の原因としては，高血圧性のものについで血管奇形によるものが多いとされている．しかし本邦でこの血管奇形からの特発性小脳内血腫に対し外科的治療に成功した報告は少なく^18，20），またこれらは慢性例で，急性小脳内血腫を血管写により診断し，原因となった血管奇形の全摘とともに血腫を除去し，救命に成功した報告はないようである．私どもは最近かかる1例を経験したので，ここに報告し，あわせて若干の文献的考察を行う．

A 38-year-old female suddenly developed pain in the back of her head and vomited several times, and soon lapsed into unconsciousness. She was admitted to our department 5 : 00 p. m. on May 8 1975, about 8 hours after the onset or the attack, for the suspected intracranial hemorrhage.

On examination on admission, she was semicomatose and reacted only to the pain stimuli. The pupils were miotic and reacted to light only sluggishly. The right eye ball deviated downward. There were hyporeflexes and extensor plantar reflexes on both sides.
