脳底動脈瘤を伴った両側対称性巨大内頸動脈海綿静脈洞部動脈瘤の1例

Ⅰ．緒言

　最大径25mm以上の巨大脳動脈瘤は比較的稀なものであり，また他の脳動脈瘤と異なり臨床的に特異な症状を呈し，外科的治療の面でも様々な問題が存在することはよく知られている．このような巨大脳動脈瘤は内頸動脈海綿静脈洞部に認められることが多いが，これが両側対称性に存在することはきわめて稀で，われわれが知りえた範囲では文献上現在までに10例を数えるにすぎない．

　われわれは一側海綿静脈洞症候群を呈して来院した症例において，脳血管撮影にて内頸動脈海綿静脈洞部に両側対称性に巨大な脳動脈瘤を認め，更に脳底動脈分岐部にも小さな動脈瘤が存在することを認めたので，ここに報告し若干の考察を加える．

An unusual case with bilaterally symmetrical giant aneurysms, arising from the intracavernous portion of the internal carotid artery, was presented.

The patient was 50 years old female. She was referred to the hospital on September 13, 1972, because of facial pain, double vision and left palpebral ptosis. On admission, neurological examination revealed decrease of visual acuity, optic nerve atrophy, paresis of the third cranial nerve and disturbance of the first and second divisinos of the fifth cranial nerve in the left side.
