Ⅰ．はじめに

　近年，硬膜動静脈奇形に対する関心が高まるにつれ，多数の報告がみられるようになった．種々の血管撮影による詳細な検索，手術所見および病理学的検討により，本疾患の病態が徐々に解明されつつある^{1-3，8，9，11，12，13，15）}．しかし，その治療方法に関しては症例により異なるが，様々な意見があり^16，20），はっきりと確立された治療法は少ない．今回著者らは，クモ膜下出血で発症し約6カ月後の脳血管撮影で左後頭蓋窩硬膜動静脈奇形と診断し本症に対して，流動性合成樹脂であるPhicon®（ψ con）^5）を用いた人工栓塞術を施行，良好な結果を得た．今回はその治療方法を中心に述べる．

　なお，著者らの渉猟しえた範囲では，本症例はこの方法を施行した初報告例である．

Many cases of dural arteriovenous malformation have been reported recently. However, the surgical treatment for these diseases has not been established yet.

We propose a new method of treatment on this peculiar vascular diseases, that is one of the artificial embolizations; A catheter is inserted into feeding artery, from which liquid silicon rubber, Phycon®, is injected. The rubber is hardened in the vascular net-work. It can not pass into drainer because of its viscosity. Therefor the malformation never develope again.
