前頭蓋窩硬膜動静脈奇形—1治験例と文献的考察

I．はじめに

　前頭蓋窩硬膜動静脈奇形を，この領域の硬膜栄養血管であるanterior ethmoidal arteryを主要なfeedingarteryとするものとすれば，充分な記載が行われた症例は現在までに13例である^{1,4,6,10,13-16,18）}．この他にも数例が散見され^3,8,14），またfeeding arteryの一部としてこの動脈やposterior ethmoidal arteryが関与している症例も報告されているが^9,17），海綿静脈洞部ないし後頭蓋窩の硬膜動静脈奇形に比較すれば稀な疾患である．

　私どもは最近その1例を経験したのでここに報告するとともに，この部位の硬膜動静脈奇形の発症形式にはその血管構成に由来する一定の特徴があると考えられるので，文献例13例と自験例の合計14例の検討に基づき，前頭蓋窩硬膜動静脈奇形の特徴について若干の考察を試みたい．

Dural arteriovenous malformations involving the anterior cranial fossa are rare. Thirteen cases with detailed description were found in the literature.

Case report: 47-year-old male, without any relevant past history had sudden severe headache, nausea and vomiting on May 20, 1979. He was refered to us on June 6. On admission, neurological examination revealed moderate meningeal syndrome with mild right hemiparesis. Spinal tap yielded bloody CSF and 220 mmH2O in pressure. Computerized tomogram revealed a high density area in the left frontal region (Fig. 3).
