Ⅰ．はじめに

　後頭蓋窩硬膜動静脈奇形（AVMと略記）は，最近報告例が増加し，成因や治療法に関する検討がなされているが，いまだ一定した結論は，えられておらず，治療困難な疾患の1つである．

　われわれは最近，脳膿瘍に対して開頭による全摘出術後2年を経過して，後頭蓋窩および後頭部硬膜AVMが発生し，2度にわたり手術を行った症例を経験したので，治療法を中心に，若干の考察を加えて報告する．

A 61-year-old woman was admitted lo our hospital because of disturbance of consciousness after onvulsive seizure. She was semicomatose state and neurological examination showed right spastic hemiplegia. Two years prior to this admission, she was admitted and an intracerebral abscess was totally removed from left temporal lobe. Angiograms revealed a dural AVM in left occipital region and posterior fossa. Feeders of the AVM were occipital arteries of both sides, left middle meningeal artery, left tentorial artery, left middle cerebral artery, muscle and meningeal branches of bilateral vertebral arteries.
