Ⅰ．はじめに

　医療とは，万人の生活を支える最も重要な社会基盤である．したがって，その質の向上は万人が求める恒久的な願いである．一方，その医療を支えるシステムは，世界に様々な人種や宗教があるごとく，それぞれの国で大きく異なっている．例えば，米国は，医療においても自由競争主義の原則を重視し，医療制度を構築してきた．英国は，国営保険制度を基盤に独自の医療制度を確立してきた．ドイツは，医療保険（疾病保険）への強制的加入を機軸に医療制度を築いてきた．一方，わが国では，1961年にスタートした国民皆保険や出来高払いの診療報酬制度，自由開業性等に特徴づけられた医療を，国が主導で展開してきた．しかしながら，よく考えると疾病に地域性があるように，医療提供体制にも地域性があってしかるべきである．今求められている医療情報ネットワークとは，こういった特徴を生かしたものでなければならない．

　そこで，本稿では，脳神経外科疾患，特に脳卒中を対象にした医療情報ネットワークのあり方を解説し，これから克服しなければならない課題を考えてみた．

　Japan is undergoing comprehensive healthcare reform including modification of medical institutes and medical circles. To success this healthcare reform we require innovative policies and newly-developed medical network based on information technology (IT) and information-communication technology (ICT). The government of Japan and local governments should rapidly enact vision for newly medical network and all medical staffs should also promote the vision to ensure that patients are very well at any time. As a result, we expect to create the new society with great potential for providing speedy patient access to the best medicines and the best pharmaceuticals to ensure that patients are in good health.
