Ⅰ．アキネジア，純粋アキネジアとは

　アキネジアakinesiaとはパーキンソニズムの特徴的な運動障害（運動の乏しさ・遅さ）を表現する包括的な症状名symptom-complexとして慣用されてきたが，その定義はいまだ十分に明確ではなく，①仮面様顔貌，②全体的なあるいは個々の動作の乏しさ，遅さ，③運動開始時の逡巡と遅延，④突進現象，⑤すり足・小刻み歩行，すくみ足，⑥歩行時の腕の振りの消失，⑦速い反復動作の障害，⑧小書症，⑨口語の障害（単調，早口で口ごもる）等々が含まれている。

　Martin^4）は主に脳炎後パーキンソニズムの臨床観察からパーキンソン症状を陽性症状positive symptoms（筋強剛と振戦）と陰性症状negative（deficiency）symptomsに2分し，陰性症状の中軸に姿勢postural mechanismの障害を据え，その延長上に種々のアキネジアと呼ばれる症状を位置せしめ，詳細な症候論を展開している。postural mechanismの求心系として歩行locomotionの解析から視覚を強調しており，その中枢として淡蒼球，線状体を想定している。

　Akinesia in parkinsonism is the symptom-complex ascribed to several motor disturbances, such as poverty and slowness of movement, mask-like facies, poor arm swinging, micrographia and freezing and festination in speech and gait.

　Martin⁴⁾ introduced the idea of positive symptoms (rigidity and tremor) and primary negative symptoms, and the latter was interpreted as the disorder of postural mechanism, being manifested as akinesia in the disease.
