2．小脳半球部—臨床症状の病変の局在—小脳半球の部分切除について

Partial resection of the cerebellar hemisphere

　後頭蓋窩の腫瘍，とくに小脳橋角部のものを手術するさい，術野の拡大をはかるために小脳半球の一部を切除することが，時々行なわれる。小脳切除の部位，範囲などは腫瘍の性状により異なるが，聴神経腫瘍の場合は比較的一定しており，半球の外側1/3の切除が頻繁に行なわれている。今回の調査は，この聴神経腫瘍手術例について，上に述べた小脳半球部分切除（cerebellar uncapping）による半球症状発現の有無を明らかにすることを目的とするが，非定型的かつより広範囲な小脳半球切除を余儀なくされた，二，三の症例についても言及し，臨床的な立場から，小脳半球切除と小脳症状との関連について知られたところを述べたい。

Partial resection of the cerebellar hemisphere is sometimes indicated at surgical removal of tumors of the cerebellopontine angle. For the purpose of investigating the effect of cerebellar uncapping, i.e., resection of the lateral one-third of a cerebellar hemisphere, evaluation of pre- and postoperative neurological status was carried out with 160 operated cases of acoustic neurinoma. Complete clinical examination of postoperative cerebellar function and comparison with the pre-operative condition was possible with 70 cases.
