目的 国立東京第二病院（東二）方式の地域医療を志向した総合診療医卒後研修方法で，どのような内容の研修が可能か検討した．
検討環境と方法 22の標榜専門科と救命救急センターからなる総合病院で，5名の卒後3年生が1年間の総合再診外来で対応した"problem"，特定の専門科に偏らないよう調整された内科系病棟研修で対応した主訴と疾患，これら患者の"profile"を蓄積して検討した．
結果 再診外来で対応した患者は，延べ約2、000名でその男女比，年齢構成は東二の位置する地域のそれらに近似した．"problem"は延べ約8,000で，循環器系，消化器系，内分泌系をはじめ「東京都通院有病者率調査」で報告されている上位30の中，眼の病気，骨折を除くすべてについて研修できた．病棟研修では内科系の疾患を中心に120疾患の研修ができた．
結論 東二方式の研修方法により，専門性診療体制を敷く国立病院においても地域医療をシミュレートした研修が可能と推測された．
Objective To evaluate the postgraduate training programe of primary care adopted by the Second Tokyo National Hospital.
Settings and method The facility consists of 22 sections of speciality and emergency care unit. Five residents (PGY-3) participated in this study. The following data in addition to patient profile were compiled for one year : content of problem managed by trainee at ambulatory care unit of the General Internal Medicine (GIM) and chief complaint and diagnosis of inpatient cared by trainee.
Results The total number of ambulatory patients were about 2000.
