　症例は66歳男性で，主訴は排尿後尿滴下と排尿困難である。耳鼻科にて副鼻腔炎手術時，導尿困難との依頼にてカテーテル留置を試みるも通常の操作では挿入困難で，スタイレットを使用し尿道力テーテルを挿入した。耳鼻科の手術後，膀胱尿道鏡，尿道膀胱造影にて球部尿道に比較的大きな憩室様所見を認めた。カウパー腺嚢胞の診断で経尿道的嚢胞壁切除術を行った。術中，カウパー管導管への開口部を認めた。術後，排尿後の尿滴下症状は軽減した。

A 66-year-old male patient with a diagnosis of Cowper's syringocele is presented. His chief complaints were urinary dribbling after urination and dysuria.Because Foley catheter was indwelt with difficulties at the operation of sinusitis he was admitted to the urologi-cal ward after otolaryngological therapy. Retrograde and voiding urethrocystography, urethroscopy revealed acystic lesion at the membraneous part of the urethra, suggesting a cyst of Cowper's gland. A transurethral resection of the cystic wall was performed. His com-plaints became decreased after the operation.
