両側カウパー管嚢胞による前部尿道憩室

A CASE OF ANTERIOR URETHRAL DIVERTICULA BY CYSTS OF BILATERAL COWPER'S DUCTS (COWPER'S SYRINGOCELE)

　血尿，頻尿を主訴とした17歳男子の前部尿道憩室の1例を報告した．内視鏡的に両側カウパー（Cowper）管嚢胞による2個の尿道憩室と診断が確定，経尿道的に憩室遠位部の弁状部を切開し軽快をみた．本症例はMaizelsらの分類における穿孔型syringo-celeとして分類すべきものと思われ，若干の文献的考察を加えた．

A 17-year-old boy was evaluated for terminal gross hematuria and urinary frequency of 3 days' duration. Retrograde urethrogram and voiding cystourethrogram demonstrated anterior urethral diverticulum (or diverticula) and 2 tubular structures, which were thought bilateral Cowper's ducts. Urethroscopy disclosed 2 diverticula in the bulbous urethra and the orifices of Cowper's ducts. The distal walls of the diverticula (distal lips) were incised endoscopically. Postoperative course was uneventful and the patient experienced powerful stream. Hematuria and frequency have resolved. Lesions of Cowper's duct assume various appearances.
