　症例は77歳，男性。1994年7月，前立腺右葉の硬結を指摘された。PSAは1994年4.6ng／ml,1995年4.5ng／ml,1996年4.5ng／ml,1997年5.0ng／mlで，触診はほぼ同様の所見であった。生検を勧めたが，拒否していた。1999年5月，呼吸困難，血痰，鼻出血のため内科入院。諸検査にてDlC，多発性骨・骨髄転移を伴った前立腺癌と診断された。PSAは4,810ng／mlで，3年間ほぼ横ばいであったPSAが，その2年後に急上昇した。

A 72-year old man had a hard nodule in the right lobe of his prostate in July, 1994. The prostate specific antigen (PSA) level at that time was 4.6 ng/ml. The PSA level in 1995, 1996 and 1997 was 4.5, 4.5 and 5.0ng/ml, respectively. The digital rectal examination revealed no change in the mean time. Prostate biopsy had been neglected by the patient until he was admitted to the Department of Internal Medicine of our hospital complaining of dyspnea, bloody sputum and epistaxis in May, 1999. Various examinations revealed prostate cancer with disseminated intravascular coagulation (DIC), and multiple bone and bone marrow metastases.
