自動縫合器，超音波凝固切開装置を使用した根治的膀胱摘除術

Application of Ultrasonically Activated Scalpel and Endoscopic Intestinal Stapler in Radical Cystectomy

　根治的膀胱摘除術において，出血量を減らすための工夫として超音波凝固切開装置，自動縫合器を使用した。超音波凝固切開装置は55,500Hzの超音波振動による機械的エネルギーにより組織の蛋白の変性，切離を行うもので，切離と確実な止血が同時に得られ手術操作全般にわたり有用であった。自動縫合器は膀胱側方靱帯の切離に使用する。血管系の処置をあらかじめ行っておけば，全く出血を認めずに迅速に膀胱側方靱帯の切離が可能であった。

We applied the ultrasonically activated scalpel and endo-scopic stapling device to control the bleeding in radical cystectomy. Ultrasonically activated scalpel (Harmonic Scalpel) is a device which can cut and coagulate tissue by converting electric energy to mechanical vibration with a frequency of 55,500Hz.Harmonic Scalpel could divide lateral pedicle, fatty peritoneum, vascular pedicle and all connective tissue safely without bleeding. We applied the stapler (ETS-Flex Endoscopic Linear Cutter) to ligate and divide the bladder lateral pedicles in radical cystectomy.
