　患者は32歳，男性。左陰嚢内容の有痛性腫脹により近医を受診した。左精巣に鶏卵大，右精巣に小指頭大の腫瘍が発見され，1989年3月6日，当院に紹介された。左高位摘除術と右部分切除術を行い，病理は左胎児性癌と右成熟奇形腫であった。術後検査はstage　Iで，PVB療法を行い退院した。術後10年が経過するが再発を認めていない。血清LH，FSHが軽度上昇し，精子数は1ml当り2〜3×10⁶個である。両側精巣腫瘍の手術法として，部分精巣切除術は今後考慮すべき治療法と思われる。

A 32-year-old man presented with a painful mass in the left scrotum. Physical examination revealed an egg-sized mass in the left testis and a palpable small mass of the right testis. The patient underwent left radical orchiectomy and right partial orchiectomy. Histology proved embryonal carcinoma of the left testis and mature teratoma of the right testis. Imaging study including abdominal CT and chest X-ray and prompt lowering of AFP level to normal after surgery confirmed the diagnosis of stage I disease. He is now leading an uneventful life without recurrence of the disease at 10 years after surgery.
