両側同時性精巣セミノーマの1例

Synchronous Testicular Tumors : A Csae Report

　患者は43歳，男性。両側同時性精巣腫瘍の診断にて両側高位精巣摘除術を施行した。病理組織学的診断は両側とも定型的セミノーマ病期1,pT1 N0 MOであった。術後補助療法として左右腸骨動脈，傍大動脈リンパ節に対して36Gyの放射線照射を施行し，現在外来にて経過観察中である。組織適合抗原（以下，HLA）検査にて両側精巣腫瘍患者に高頻度に認められるHLA-A24が認められ，HLAの関与が推測された。

A 43-year-old man was admitted to our hospital with complaint of the left testicular tenderness and nodule in February 1997. Under the diagnosis of bilateral testicular neoplasm, bilateral high orchiectomy was performed. Histological examination revealed typical seminoma of bilateral testis, stage I and post-operative irradiation was performed. Now the patient is doing well without any sign of metastasis. Genetic factors as human leukocyte antigens (HLA) , which have been shown to be important in the development of bilateral testicular tumors, was positive in our case.
