膀胱腫瘍の新分類—特に従来の分類との比較

MORPHOLOGICAL CLASSIFICATION OF THE URINARY BLADDER TUMOR BASED ON THE INTERRELATION BETWEEN BEHAVIOURS OF STROMAL AND PARENCHYMAL PROLIFERATIONS

はじめに

　先にわれわれは間質と実質の相互関係に基づいた新しい膀胱腫瘍の分類を提案した^8,9）。この分類について，特に泌尿器科の専門医の方々から，多くのご批判やらご質問を戴いた。この分類は組織構築学的分類に臨床的予後を踏まえた分類を加味したものであり，従来の分類とはその発想が異なるものであるが，この分類には多少の混乱もあり，考えなおすべき点にも気がついた。そこでこの分類を再び検討し，より理解しやすいように，そして泌尿器科の臨床，特に内視鏡的な所見と総合的に対照して理解しやすいように修正した。

　本論文では，1．なぜこのような分類を試みたか？　2．部分的に修正したこの分類の基準とその内容，3．従来用いられている分類との関係について改めて整理して記載したので，大方のご批判を再び仰ぎたい。

Previously, we presented a new classification of the urinary bladder tumor according to its histo-logic architecture involving the tumor cells and fibrovascular stroma. This classification was made to understand easily of the histologic architecture of the tumor related to both the endoscopic findings and clinical prognosis of the patients. For more satisfactory achievement of the purpose in the previous classification, the slight modifications were performed as follows:

1) The major types of bladder tumors are classified into transitional cell tumor, squamous cell carcinoma. adenocarcinoma and others.
