はじめに

　膀胱癌の大部分は乳頭状発育を示すものであり，膀胱鏡検査で腫瘍を認めることにより，その診断はさほど困難ではないと考えられてきた。しかしながら，数年前から，頻尿，排尿痛といつた慢性膀胱炎の症状を呈し，膀胱鏡検査では，はつきりした腫瘍の認められない膀胱癌の存在が報告され，carcinoma of the bladder simulating chroniccystitisと呼ばれている^1）。このような腫瘍は，初期には上皮内癌の形態を示し，速かに浸潤癌に移行するものと考えられることから^2），早期診断はきわめて重要である。われわれは6例の上皮内癌と，7例の上皮内癌より進展したと考えられる浸潤癌を経験し，その発見には，尿細胞診がきわめて役立つたので報告する。

Six patients with in situ carcinoma of the urinary bladder and 7 patients with invasive carcinoma developed from in situ carcinoma were experienced. The symtoms of these patients, such as pain on urination and urinary frequency, suggested chronic cystitis. Cystoscopic findings were not pathognomonic. However, in all cases cancer cells were detected by routine cytological examination of the sediment for patient's voided urine.

The characteristic features of these malignant cells in situ carcinoma included none of necrosis and a uniform pattern of relatively less malignant cells with nuclear atypia of slight degree.
