尿細胞診の臨床的意義

THE DIAGNOSTIC VALUE OF URINARY CYTOLOGY

緒言

　近年，細胞診の適用域は著しく拡大され，その目的はスクリーニング，診断ならびに治療効果判定に大別される。泌尿器科領域でも職業性膀胱癌のスクリーニングや上皮内癌の診断，膀胱癌の保存的療法後の経過観察などの目的に広く活用され，その臨床的価値が再認識されるようになつてきた。今回，われわれは秋田大学医学部附属病院と関連病院の泌尿器科における過去5年間の尿細胞診を集計し，その有用性について検討を加えたので報告する。

Cytological analysis of 2,567 urine specimens were reported. The cytological findings were compared with the clinical and histological diagnosis. The positive rates in renal cancer (n=34), pelvic cancer (7), ureter cancer (17), bladder cancer (124) and prostate cancer (21) were 8.8%, 28.6%, 47.1%, 51.6% and 14.3%, respectively. The false positive rate in class Ⅳ and V was 5.5%. In patients with class Ⅲ, the rate of urinary malignancy was 34%. The result suggested taht urinary cytology should be repeated in this class.
