はじめに
陰茎持続勃起症は必ずしもまれな疾患ではないが，治療法が確立されていない現在本症に遭遇して戸惑うことが多いのではないかと思われる。近年本症に対し静脈副血行路造設法としてGrayhackら1）の陰茎海綿体大伏在静脈吻合法やQuackels2）の陰茎海綿体尿道海綿体吻合法が行なわれているが，術後しばしばimpotenceを来すという欠点がある。ところが1975年Ten Cateら3）は静脈副血行路造設法の一方法としてexternal corporovenousshuntを発表しているが，本法は上記二方法に比較して手技が簡単であり施行例のすべてにimpo-tenceの発生をみなかつたことなどから優れた治療法と思われる。今回，私たちは特発性陰茎持続勃起症と思われる1例にこのexternal corporo-venous shuntを追試する機会を得たので二，三の検討を加え報告する。
A case of idiopatic priapism treated by external corporovenous shunt described by Ten Cate et al in 1975 was reported. This method has many advantages: simple surgical procedure, easy to check the patency of the shunt, and removal of the shunt after confirmation of normal venous return by corporo-gram taken easily through the needle placed into the corpora for the purpose of drip infusion of heparinized saline (1000 units/500 ml). This regimen offers one of the most effective treatment for priapism.
At present two months after operation, normal erection is not gained but is expectedly followed.
