　76歳男性。2007年5月中旬左陰囊腫大を主訴に当科を受診した。左精巣腫瘍の診断にて左高位精巣摘除術を施行した。病理診断は脱分化型脂肪肉腫であった。高悪性度であったが，適切な切除縁により局所の根治性を高めることができた。術後1年10か月経過現在，再発徴候を認めていない。陰囊内脂肪肉腫は稀な疾患であり，術前に精巣腫瘍や鼠径ヘルニアと診断してしまうことが多い。陰囊内脱分化型脂肪肉腫は特に頻度が低く，本邦で11例目である。脱分化型脂肪肉腫は予後不良である。脂肪肉腫の治療法は外科的摘除が第一選択であり，適切な切除縁により局所の根治性を高めることが重要である。リンパ節郭清は意味がないとされている。精索脂肪肉腫は局所再発が多く，遠隔転移は稀である。今後も注意深く経過観察していく必要があると考えられた。

　A 76-year-old man with the chief complaint of a painless left scrotal swelling,was diagnosed as having testicular tumor and then received the left high orchiectomy. Histopathological diagnosis was dedifferentiated liposarcoma. The patient is well and has no evidence of recurrence for one year and 10 months following the operation. Dedifferentiated liposarcoma occurring in the scrotum is rare. With our knowledge,only eleven cases,including our case,have been reported in Japan. The prognosis of the patients with dedifferentiated liposarcoma is considerably poor. For better prognosis,the surgical resection of tumor with a sufficient surgical margin is mandatory. The local recurrence is more commonly seen than the distant metastasis in the patients with liposarcoma,therefore the patient needs further strict and long-term follow up.
