短期間に増大傾向を認めた陰囊内平滑筋肉腫

Leiomyosarcoma of the scrotum

　症例は68歳，男性。1年前に陰囊内の腫瘤を自覚し，徐々に増大したため受診した。初診時，左陰囊に22mm大の弾性硬，表面平滑，可動性良好な腫瘤を認めた。精巣との連続性は認めず，超音波所見は内部不均一な低エコー腫瘤であった。腫瘤摘除術を施行し，組織学的に陰囊壁原発の平滑筋肉腫と診断した。切除断端陰性で，全身検索上転移を認めず，術後経過は良好である。陰囊内平滑筋肉腫は比較的稀で，本邦では24例目の報告である。

　A 68-year-old male patient presented at our hospital with painless left scrotal mass which gradually enlarged over the previous 1 year. Physical examination revealed an elastic hard and round mass,measuring 2.2 cm in diameter,which was isolated from testis,epididymis and spermatic cord. Ultrasonography showed hypoechoic and multilocular mass. We performed surgical resection of the mass and the histological examination confirmed leiomyosarcoma. Surgical margin was negative and therefore adjuvant therapy was not performed. No recurrence was observed at 5 months follow-up after the operation.
