骨肉腫像を呈した脱分化型脂肪肉腫

Dedifferentiated liposarcoma with osteosarcomatous differentiation

　症例は60歳，女性。2008年4月に右下腹部痛と腹部膨満感を主訴に近医を受診し，右腎腫瘍として紹介された。CTで右腎下極に最大径10cmの石灰化を伴う腫瘤性病変があり，腎腫瘍ないし後腹膜腫瘍として，腫瘍と一塊にして右腎摘除術を施行した。病理組織上骨肉腫像を呈した脱分化型脂肪肉腫と診断した。脱分化型脂肪肉腫は，稀な腫瘍で予後も不良といわれているが，骨肉腫像を呈することはさらに稀で，詳細はほとんど不明である。

　A 60-year-old woman with complaints of right lower abdominal dull pain and abdominal fullness was referred to our hospital with the diagnosis of right renal tumor by the computed tomography on April,2008. CT scan showed a tumor with calcification measuring 10cm in maximum diameter,in the lower pole of the right kidney. Under a tentative diagnosis of renal tumor or retroperitoneal tumor and translumbar radical nephrectomy was performed. The histopathological examination revealed retroperitoneal dedifferentiated liposarcoma with osteosarcomatous differentiation. Retroperitoneal dedifferentiated liposarcoma is extremely rare with poor prognosis discribed in the literatures.
