　症例は34歳，女性。右背部痛を主訴に近医を受診，CTで右腎周囲に血腫が認められ当院を紹介され，緊急入院となった。若年女性であり，血管造影にて腫瘍内に微小動脈瘤が認められ，腎血管筋脂肪腫の自然破裂と診断し塞栓術を施行したが，腎摘除術は患者が拒否したため施行しなかった。経過観察中のCTにて，腫瘍は著明に増大し偽被膜が認められ，腎細胞癌が疑われたため，右腎細胞癌T2，N0，M0と診断し右肋骨弓下横切開にて根治的右腎摘除術を施行した。組織学的に平滑筋成分や脂肪成分はみられず，淡明細胞癌と診断した。画像上，腫瘍内に明らかな脂肪は認められなかったが，血管構造が豊富で腎血管筋脂肪腫との鑑別が非常に困難な症例であった。自然破裂した腎細胞癌の予後に関しては一定の見解が得られていないが，腫瘍細胞の播種の可能性もあり，厳重な経過観察が必要であると考えられる。

　A 34-year-old woman with the chief complaint of right backache was hospitalized and the right perirenal hematoma was found on computed tomography（CT）. Microaneurysms were found in the tumor on an-giography. A diagnosis of spontaneous rupture of angiomyolipoma was made. We only performed transcatheter arterial embolization,because nephrectomy was refused by the patient. As the tumor increased markedly in size,with a pseudocapsule found on CT during follow-up,a diagnosis of renal cell carcinoma was made,and we performed radical nephrectomy. Histologically neither smooth muscle nor fat components were observed,and the diagnosis of clear cell carcinoma was made. Prognosis of spontaneously ruptured renal cell carcinoma is not determined yet,therefore careful follow-up is required.（Rinsho Hinyokika 60：1007-1010,2006）
