前立腺癌に対し，下腹部7cm切開のミニラパロトミー法による恥骨後式逆行性前立腺全摘除術を10例に行い，従来法20例と比較検討した。ミニラパロトミー法では手術時間が平均3時間21分と，従来法の2時間11分に比べ有意に長いものの，CRPに代表される術後の負荷には差がなく，反面術後の鎮痛剤の使用量は有意に減少していた。また，術後の尿失禁の治癒までの週数には有意差はなかった。合併症として1例で直腸損傷がみられたが，術野の展開が不良であったことが誘因になったと考えられた。したがって切開創を小さくすることにより低侵襲の術式となるものの，ミニラパロトミーで良好な術野が得難い場合には速やかに切開を拡張するべきである。

Retropubic radical prostatectomy was performed via a lower abdominal laparotomy（7cm long incision）in ten patients.The results were compared to those in 20 patients who underwent conventional radical retropubic prostatectomy.The average operating time was 3 hours and 21minutes in patients with minilaparotomy,which was significantly longer（2 hours and 11minutes）compared to the conventional method.Operative invasiveness represented by postoperative CRP showed no difference but less analgesics were necessary in patients with minilaparotomy.There was no significant difference in the recovery time from postoperative urinary incontinence.Rectal injury occurred in one minilaparotomy patient due to an insufficient operating field.Therefore,the abdominal incision should be extended when good view is difficult to obtaine by minilaparotomy.
