症例は54歳,男性。無症候性血尿を主訴に近医を受診し,膀胱結石と前立腺肥大症を指摘された。経尿道的膀胱砕石術を施行した際,後壁に粘膜下腫瘍を認め,当科を紹介された。経過観察していたところ増大傾向を示したため,手術目的で当科に入院した。経尿道的膀胱腫瘍切除術を施行したところ,病理組織学的所見は膀胱パラガングリオーマであった。今後も十分な経過観察が必要である。
Pheochromocytoma of the urinary bladder is an unusual tumor that typically presents with hypertensive crises related to micturition.We describe a case of bladder pheochromocytoma in a 54-year-old male in which macroscopic hematuria was the only alarming symptom.Cystoscopy and computed tomography revealed a submucosal bladder tumor.Complete transurethral resection was performed without any intraoperative complications.The histopathological diagnosis of paraganglioma was confirmed by immunohistochemical stainings.The postoperative course was uneventful.Careful observation is mandatory because of possible recurrence or metastasis of the tumor reported in the literature.
