著明な胸水貯溜を伴つた薬疹の1例

DRUG ERUPTION ASSOCIATED WITH MARKED ACCUMULATION OF PLEURAL EXUDATE

　いわゆる重症薬疹には，肝，腎，血液障害がよく合併することは周知のこととされている．しかし，肺の所見に関しては薬疹とは別に薬による気管支喘息やレフレル症候群などが知られているが，薬疹に伴う肺の異常所見に関しては，その報告は比較的少ない．著者は最近，著明な胸水貯溜を呈した薬疹を経験したので以下に報告する．

A 22-year-old man showed oozing exudative erythemas of up to 5 mm in diameter on the entire body surface after administration of large doses of sedatives and antifebriles for a cold.

Laboratory studies revealed proteinuria, marked leukocytosis, eosinophilia, 4 plus CRP, hypopro-teinemia and abnormal liver functions (alkaline phosphatase, GOT, GPT and LDH).

It should be emphasized that a marked accumulation of pleural exudate was found on the right side. The exudate disappeared without any special threatment, as the cutaneous manifestation cleared.

The patient was treated effectively with topical steroid application. The accumulation of the pleural exudate seems to be very rare as a sign accompanied by drug eruption.
