　はじめに

　外毛根鞘癌（trichilemmal carcinoma）は比較的稀な毛包系悪性腫瘍であるが，その中で頭頸部は好発部位である^1）。本腫瘍の多くは比較的早期に皮膚科で治療され，上皮内にとどまっているものでは外科的切除により予後は良好とされている^2）。そのため耳鼻咽喉科の日常臨床で遭遇することはほとんどない。

　今回われわれは右耳下部に発生し耳下腺に浸潤した同腫瘍症例を経験したので，その臨床像，治療経過について報告する。

A 66-year-old Japanese male was complaining ofright subauricular tumor that invaded the right parotid gland. He underwent enbloc resection of the tumor with total parotidectomy and right radical neck dissection. Histopathological examination revealed trichilemmal keratinization regarded as a typical finding of trichilem-mal carcinoma. He died of lung metastasis 6 months postoperatively. Surgical extirpation of the tumor in an early stage is highly recommended, since it has a ten-dency of external growing.
