　はじめに

　進行耳下腺癌は外耳道，下顎骨，側頭骨，顎関節などの耳下腺をとりまく解剖学的な問題があるため，大きく病変部位を切除するのは困難なことが多い。今回われわれが経験した2例はいずれも再発例で，外耳道前下壁から翼突筋，乳様突起に至る部分に癌の浸潤を認めた。頭蓋底，内頸動脈には浸潤がなかったため，乳突蜂巣を削開して骨部外耳道，下顎関節包，頬骨，翼突筋とともに腫瘍を一塊に切除できた。この2症例の手術経験について報告する。

We discussed the management of two cases of recurrent malignant parotid gland tumor. The par-otid gland is in close approximation to the externalauditory canal, temporal bone, mandible and tempero-mandibular joint. In light of this anatomi-cal location of the parotid gland it is rather difficult to resect recurrent malignant parotid gland tomors completely. We performed extended total parot-idectomy with the partial resection of the mandible, temporal bone, and zygomatic bone, and recon-structed the defect using a rectus abdominis mus-culo-cutaneous flap.
