耳下腺基底細胞腺癌の1例

A case of basal cell adenocarcinoma of the parotid gland

基底細胞腺癌は，1991年にWHOの耳下腺腫瘍組織分類第2版で初めて独立して分類された比較的新しい概念の腫瘍である^1）。これまで，報告例は少なく唾液腺腫瘍のなかでの発生頻度は約1％前後とされている^2）。本腫瘍は良性腫瘍である基底細胞腺腫と組織学的にも類似点が多く^3），一般的には低悪性腫瘍と考えられている。腫瘍の増大は通常緩慢で，顔面神経麻痺をきたすことは稀とされている^11,12）。今回われわれは，顔面神経麻痺をきたした耳下腺原発の基底細胞腺癌の1例を経験したので報告する。

Basal cell adenocarcinoma（BCAC）is a newly categorized entity in the second version of the World Health Organization classification of salivary gland tumors. A 64-year-old male with BCAC of the parotid grand presenting a left parotid mass,was treated with surgical excision and radiotherapy. We summarized 43 cases of BCAC of the salivary glands reported in Japan. Although it has been suggested that the prognosis of BCAC is good,long-term follow up is mandatory because of relatively high rate of local recurrence.
