喉頭全摘を施行した眼・咽頭筋ジストロフィーの1症例

Oculopharyngeal Muscular Dystrophy Treated by Total Laryngectomy:A Case Report

　はじめに

　嚥下の動的障害をきたす疾患としては，筋萎縮性側索硬化症，重症筋無力症，多発性筋炎などがある。これら疾患の多くは顔面，体幹，四肢の筋が早期に侵されるため，嚥下障害は一部分症状としてみられることが多い。一方，嚥下障害を主症状とする筋原性疾患の1つとして眼・咽頭筋ジストロフィーoculopharyngeal muscular dystro-phy （以下，OPMD）が挙げられる。今回われわれはOPMDによる嚥下障害に対して，喉頭全摘術を施行し嚥下困難の改善をみた症例を経験したので若干の文献的考察を加えて報告する。

A 63-year-old male complained of dysphagia. On examination, bilateral ptosis ophthalmoplegia, soft palate palsy and marked salivary retention in the hypopharynx were noted. The barium swallowing examination revealed misdeglutition with retention of barium in the pyriform fossa.

Cricopharyngeal myotomy did not resulted in relief of dysphagia, because swalling pressure was remarkably reduced.

Dysphagia was improved by the subsequent opera-tion of total laryngectomy with successful result.
